downloaded by the aspiring client and given to his/her
veterinarian or directly by the Veterinarian wanting to
refer their patient to rehabilitation.
The referring Veterinarian is to fill out
referral form and fax to our facility or send along with client to
present during their consultation exam with Dr. Wall.
Another way to aid our staff in setting up
the appropriate exam for your pet, is by filling out a "new" client or "current" client information form, then submit
it. By doing this, our staff will be fimiliar with your pets needs
and current situations. And as a "new" client, your information
will already be entered into our system, saving time on
arrival.
This will help to acknowledge the
patient's history of existing medical situation, as well as
the patient/client relationship between the
patient's current Veterinarian and our facility's Veterinary
staff for rehabilitation.
